2009-2010 PACKET CONTENTS AND CHECK LIST

This packet should contain the following:

1. FERPA/LANGUAGE SURVEY
2. ACTIVITY FEE NOTICE
3. ATHLETIC TRYOUTS INFORMATION (Do not mail any athletic information. The athletic office will be open Aug. 24, 25, and 26th from 9:00am - 12:00 for sign ups - physicals, athletic fees, permission slip/emergency card are due at this time.)
4. HEALTH INFORMATION FORM
5. MATH/SCIENCE DEPT. CALCULATOR REQUIREMENTS NOTICE
6. MENINGOCOCCAL VACCINE INFORMATION
7. LSPO Summer Communication - LS Student Directory Update information, LSPO Email Notices sign up, Volunteer Sign-up, Annual Gift to the LSPO
